PS It is not true that xTOC styles have to be updated on the reference
pages. You can update the style using the Paragraph Designer anywhere,
including body pages. To change the content of the xTOC entry, however,
you have to modify the building blocks on the reference pages. Also, if
one style updates, but not the subsequent ones, it is likely that you
did an Apply rather than an Update All.
