Page numbers for index main entries sometimes disappear

I believe you have to have an explicit marker for the main entry.

foo:bar

results in

foo
   bar   27

foo;foo:bar 

results in

foo   27
   bar   27

When I add subentries, the page numbers disappear sometimes for the main
index entries. What causes this and how can I fix it? Thank you!
