Trouble with paragraph tags in TOC

That's the strange part. The tags are listed in the Setup dialog along with the ones that are working correctly. 

BTW, I'm using FM 7.2 on Windows if it makes a difference.

Have you specified those tags in the Set Up TOC dialog for the
book file?  That's really the essential step.  Unless the tag is
specified in the Set Up TOC dialog, FrameMaker will ignore it
when updating the TOC regardless of whether there is a
prototype paragraph on the TOC reference page.
