Thank you.  Yes, that was part of the issue:  the first file in the book has 
a special table in the reference page.

The other issue is a random Frame9 bug, which I will probably have to just 
work around.

The following is for Frame 8 but I would imagine that it is similar for
Frame 9.

For the .html conversion on the book level, you have to use the
Reference Page in the very first file in the book. As you scroll down,
you should notice two sets of html conversion tables. One is on the
'HTML' pages and the other, at the very bottom, is on the 'BookHTML'
pages. The second one sets the conversion for the whole book.

I'm pulling my hair out.  I can't get Frame 9 (when it is not crashing
for random reasons) to output my Structured book as multiple HTML files
(one each chapter).

I'm being stupid about generating the HTML, just doing Save Book As
(selecting HTML).

I set up all my structured chapters using the difficult-to-find HTML
Setup menu window.  My element called Chapter (this begins a new
chapter) is mapped to a Division and I have selected Start New, Linked
Web Page at levels (all levels).

That's supposed to break up the HTML file and a new web page when it
comes across the Chapter element.  At least, I think it is.

But it does not do that.  It just outputs one vast HTML which consists
of all the chapters in the book.  As a bonus irritation, all the links
in the book assume/think that the book has been split and look for the
split HTML's.  But they were not generated.

My HTML conversion table says this:

E:Chapter | DIV | 1,2,3,4,5,6,7,8,9 | N |

this is:

FM Source | Element | New web page? | include auto# | Comments

My HTML conversion table for paragraphs does not include any "yes" items
since it is structured and generating HTML from elements alone.

Or do I also need to alter how the paragraph styles are mapped as well?

Ugh!
