Then, if you aren't using Frame 8, or greater, about the only way you can get what you want is by using conditional text, which is what Frame 8 really uses.
 
Just set up a condition code for new/replaced and old/changed, or whatever names you want to call them. Then distribute with the condition(s) tuened on.

Could you clarify your question? I'm reading it two ways:
Either you need to flag changes in FM since the previous rev so they stand out to reviewers. OR... you want to let people record comments in the text, for which others are recommending (correctly) to use Acrobat to track comments.
