How do I create a cross reference which uses more than one lineof text?

Hi Shmuel,

You're right, should use two formats, but the best solution is suggested by
Shlomo. He shows it can be done with one cross ref.

I think you need two separate cross-references with two cross-reference 
formats, the second one including the page.
