HTML Output + Templates and master pages

Thanks to all of your contributions on my requests for advice on HTML
output plus templates and master pages. There is a lot of good material
there and some of you guys have obviously gone to some effort. Now all I
need to do is find time to sit down and digest your advice amid all the
other pressures here.
