documentation request form?

I've been tasked with researching practices related to documentation requests. Currently, we have no formal process and I suggested that requiring requestors to fill out some sort of form would enable our department to quantify staffing requirements and provide more accurate insight into when resources will become available. Do any of you have suggestions/templates to share? TIA
