SOLVED Re: PDF bookmarks anomaly

Three responses privately from Roger Shuttleworth, Samantha Lizak and Penelope Perkins and you were all dead right.  Somehow I'd corrupted the flow in chapters 37 and 38.  When I popped them both into clean templates, problem solved!
