Missing title bar, disappearing Application bar

Thanks for asking.  I failed to mention.  Yes, I have reset my workspace numerous times.  Several times, I even tried saving my workspace without the "Application" bar displayed.  All to no avail.

Dave,

Have you tried resetting your workspace (from the pulldown to the left of the minimize/maximize/close buttons)?  I'm on unstructured FM10, so can't test.
