[Framers] Thumb Tabs

I agree with Steve. I did a few books like this unstructured. I can't remember how I did it but I found the instructions somewhere online.

Yes, and yes. However, I would suggest you use thumbs tabs on every page, and move them down the page as the chapters increment. This makes them easier to use. I can tell you how to do this (automatically, unstructured) if you decide to go down this route.
