The way I solved this was to make sure that the book had a real TOC

Now that I know the fix, I'm sure that understand the reported symptom. I wasn't sure at first. If I remember correctly, I had a similar problem some years ago with unstructured FrameMaker files. I don't recall that I ever figured out how I (or what) caused the problem.

Thanks, Stephan, for the link. My book is structured and I was picking up elements in the TOC. I reconfigured the TOC to pick up paragraph formats instead and set the indents like you suggested and now it works! Thanks to all who responded and thank you to Stephan for the help.

This is related to the TOC in your book. I have noticed this a couple of times. We are currently investigating this.

FrameMaker 2015's Publish Feature. The TOC pane is getting populated with
the file names from my book instead the TOC entries. How do I get my TOC
entries to show in the TOC pane on the left?
