[Framers] Handling very long TOC entries

My TOC looks great, except for entries that are so long as to almost
touch the page number.  This makes the entry harder to read and it
also eliminates the dot leader.  What's the best solution for this?
I've tried adding a right indent, but this puts the page number at the
left indent position on the following line.  It helps if I add another
tab, but I can't get the page number to the right of the right indent.

What's the best practice for this situation?
