[Framers] Generated files and master page paragraph tags

I believe you have to have the titles on the body pages.
You may know this already, but to make an Index or ToC easily from an
existing one, you can just copy the existing one into the same folder as
the book file, then rename it the same as the book file itself. Thus if
your book is myBook.book, the Index file is renamed to myBookIX.fm and
the ToC is myBookTOC.fm. When you generate these files from the book, FM
will pick up the renamed file with all its formatting and repopulate it
with the relevant items.
This saves creating one from scratch and formatting the different
levels, etc. But note:
* It only works if you use a generated file as the renamed source file.
* The file name is case-sensitive and must match the book file exactly.
* IX and TOC in the file name must be upper case.
You probably knew that already!
