Bleeding Headers/Footers

First of all, many thanks indeed to Art and Fred for their advice on runaround text.
 
I am afraid that I have another question, though, which regards bleeding headers and footers.
 
Could anyone kindly tell me how you can get a coloured header (by coloured I am referring to background colour) to bleed into the top of the page (effectively extending the colour beyond the header text frame)?
