TOC pages start at page "4", and I want it to start at page "iv".

>From the book file, right-click on the TOC file, then select Numbering from
the context menu. On the Page tab set the numbering to lower-case roman.
After setting the numbering properties, remember to do an Update Book for
the changes to actually get applied.
