hyperlink problem - solved

Thanks Art and Stuart,

I needed to turn on the 'Create Named Destinations for All Paragraphs'
option.
I have a feeling this issue has come up before, but I'd forgotten about
it! 

BTW - I'm printing a postscript file and then distilling, because it
seems to be the only way to get the book into a single PDF file. If I've
missed something, please let me know.

Also try turning on Create Named Destinations for All Paragraphs, on the
Links tab.  That will initially bloat your pdf file, but you can
optimize in Acrobat to remove unused named destinations.
