Generated TOC entries appear in wrong chapter

Tori,

Thank you--your answer was spot-on! All I had to do was select each
chapter's text box containing the chapter heading, right-click, and choose
Send to Back. Now my TOC is happy and so am I.

Mary,

  I've seen this happen when the text box containing the chapter heading 
is a different box than the rest of the text. I forget if it needs to be 
front-most or back-most to be first, but it's one of the two.

If, on the other hand, this is all in one text box, I'm stumped.
