As far as:
"Word has had a bug since 2.0 such that if you
put a hard page break directly in front of a heading and then
cross-reference to that heading, Word inserts a hard page break in
front of the cross-reference."

In Word, a cross-reference refers to a bookmark. A bookmark is a 
selection of text. When the curser is deirectly before a bookmark, 
anything you type or paste is included in the bookmark. That is, the 
bookmark is expanded to include the new text. When using Word, you get 
used to not pasting anything directly before something that may be 
bookmarked.
