You should have FM automatically create the TOC file and then import
your page layouts from the chapter file into the TOC, then modify the
TOC styles as needed to fit that layout. That's it.

The reason "nothing is working" is likely because per your process the
TOC file in your book contains no TOC elements on the reference pages
for FM to use in building a TOC. Best practice is to import layouts
into a generated TOC file.
