Thanks to all for the suggestions, all of which I tried.

I still don't know why, but when I removed the "book-within-a-book" it all worked fine. I shall investigate further, but have Plans B to Z in case I can't find a proper solution.

I now need a lie-down in a darkened room, especially as the country will grind to a halt tomorrow because of some wedding or other. More beer, I think.

Hi Cynthia,

Bookmarks are only created, when "Generate Acrobat Data"
in the print dialog is activated. Possibly this option was activated when you printed the index but not for the large book.

Baruch advised to add GroupTitlesIOM in Format > Document PDF Setup > Bookmarks. Did you select all files in the book, before you did this?
