TOC formatting problem

Steve, please check the ¶ format of title.0TOC on the reference page. It may have a local override 
saying 'plain' while in the ¶ catalogue there is bold.
For the generated file only the catalogue is relevant. What's on the reference page is just 
'generation pattern'.
