screwy ToC numbering

One of my ToCs has decided to give me trouble just before a release. Unlike 8 other books, this one has:

*         Chapter 1 followed by five Chapter 2s even though they are all numbered properly in the book,

*         Page numbers in lower-case Roman numerals instead of Arabic numerals

Everything is normal in the book, it's only the ToC that causes problems.
I appreciate any insights!
