Table numbers don't continue in book

Have you done a Generate/Update since changing the settings? 

Do the paragraphs containing the figure/table titles have overrides? 

Are the paragraphs inside a tagged text flow? I've seen instances where
paragraphs inside a text box don't always update properly or in the
correct order.
