But as soon as the reader switches out of single page view (e.g., changes zoom), the behavior Pete doesn't like returns, because the bookmark destination is still the heading. 

Pete, your lengthy procedure can be shortened somewhat. But I don't know of any other (that is, automated) solution unless one of the TimeSavers tools from Microtype.com provides one. 

Here's the simpler manual process: 

1. Right-click the chapter bookmark and select Properties, and then select the Actions tab. 
2. Under Actions, click Edit. 
3. In the dialog, select Use Page Number, and set Page to the correct number.
4. Set Zoom to Inherit Zoom and click OK. 

That replaces the first dozen steps of your procedure.

As for steps 13-16, well -- the best solution is to use an autonumber in the title pgf for the chapter/appendix numbering, not a separate pgf. 

I can't think of any elegant alternative, but maybe you could kludge something with an autonumber (or xref) using a char format that makes the number very small and white.
